Abstract:
Adhesive capsulitis is characterised by progressive restriction of shoulder functions affecting both active and passive range of motion. There occurs the fibrosis of the capsule and synovial inflammation of shoulder joint. Manipulation under anasthesia remains one of the topics of clinical interest as it is still not completely understood. This study aims to evaluate the effects of manipulation under anaesthesia for adhesive capsulitis. METHODS: this study was hospital based prospective interventional study done in GMC Jammu where 37 patients were enrolled (17 Males and 20 females) from OPD after diagnosing them as adhesive capsulitis. These patients were refractory to at least 3 months of conservative treatment with 6 months of shoulder pain. All Patients underwent structured physiotherapy after the manipulation under anaesthesia as per the protocol. The recovery was documented by using the SPADI index at different follow ups. RESULTS: At subsequent follow-ups the SPADI index showed significant improvement with a decline from 0.773 pre procedure to 0.259 at 6 months with (p<0.0001). At every follow ups there was a statistically improvement in functions of shoulder joint (F=272.808, p<0.0001). additionally the age and gender showed no significant association with the outcome where as it was found that the patients without comorbidities showed better improvement in functions of shoulder joint as compared to the ones suffering from various comorbidities. CONCLUSION: Manipulation under anasthesia is one of the most favourable and effective treatment modality for the patients of frozen shoulder whereas the outcomes are independent of the demographic factors like age and gender but the systemic comorbidities significantly affects the outcomes.
